Reducing Health Disparities Through Increased PAD Awareness, Identification, and Management (1.0 CE, 0.25 Pharm)

1. Describe pathophysiology, common signs and symptoms of PAD
2. List the major risk factors and health disparities related to PAD
3. Review guidelines-based strategies, potential barriers, and solutions for PAD diagnosis
4. Outline non-pharmacologic and pharmacologic strategies for the management of PAD

Prevention and Treatment of Venous Thromboembolism (VTE) from Hospital to Home (1.0 E, 0.25 Pharm)

1. Review the major risk factors for VTE blood clots
2. Describe best practice clinical strategies to prevent VTE in hospitalized, medically complex patients
3. List the antithrombotic medications approved to prevent and/or treat VTE blood clots
4. Review three strategies for improved adherence to medication through transitions of care

Telehealth, Remote Patient Monitoring, and Virtual Disease Management for Atrial Fibrillation: Where do we go from here? (non-CE)

1. Explain current utilization of telehealth and consumer wearable devices to improve patient access to appropriate care and ways to increase clinician confidence with application in practice
2. List barriers to the adoption of telehealth for health care professionals and patients
3. Decipher what the future of telehealth and consumer wearable devices may look like after the COVID-19 pandemic
