Hypertension Screening Toolkit

Designed to screen for undiagnosed hypertension in community settings such as senior centers. This digital toolkit includes:

• Guidelines for events, whether stand-alone or in conjunction with other community activities
• Planning and implementation steps, including the screening process
• Links to PCNA’s Screening Event Toolkit

PCNA members may borrow electronic blood pressure monitors to support their screening events. See the PCNA Screening Event Toolkit for more information.

The separate screening results form can be completed during the screening and sent with the individual, along with recommended next steps, such as following up with a primary healthcare provider.
