Eryn Bryant practices at The Ohio State University Medical Center in the ambulatory heart failure clinic. Her career has centered on building and scaling multidisciplinary, system-wide programs aimed at improving cardiovascular and quality outcomes.

At Ohio State, she is the Advanced Practice Lead for the Heart & Vascular Transitions of Care Program, providing post-acute support for heart failure and cardiac surgery patients across the health system, and is Co-Founder of the Heart Wellness Clinic – which offers tailored lifestyle assessment, education, and support to individuals with established CVD or heart failure. She was the 2024 recipient of the OSU APP Innovation Award.

Prior to Ohio State, Eryn practiced at Stanford across the general cardiology, women’s heart health, and preventive cardiology clinics. There, she co-developed a nurse practitioner-led cardiometabolic program and served as a research coordinator for several CV prevention-focused clinical trials.

Eryn is a PCNA Fellow and currently serves on the Board of Directors as Treasurer, sits on the Advocacy Committee, and previously chaired the Continuing Education Committee.
